Let’s get started on this adventure by first getting really clear on the specific outcomes that you want to achieve. I’ve provided some revealing questions below to help you determine what it is that you really want for your life.

These questions will help you to create a workable path and get you going to actually realizing your greatest wishes and dreams.

Do your best to answer these questions as clearly and thoughtfully as possible, expressing the best of who you are.

These are “pondering” questions designed to stimulate your thinking in a particular way that will make our work together even more productive.

I suggest that you take several days to compose your responses to these questions. I'd highly recommend that you revisit this questionnaire each month. This way you will be able stay focused on the results you want to achieve.

What accomplishments do you think must occur during your lifetime so that you will consider your life to have been satisfying and well lived?

Now, let’s look at nine sections that, together, represent one way of describing a whole life. Here we’ll measure your level of satisfaction in these nine areas.

This is not a picture of how it has been in the past or what you want it to be in the future. It is a snapshot taken in this moment.

The emphasis is on your level of satisfaction in each area. For each area below, rank your level of satisfaction with each life area by rating it from 0 to 10 ( 0 as being poor and 10 as being ideal)

Now from this list, identify one or two areas that you want to hold as your main focus. For each focus area, provide a simple heading and a description of a measurable result.

For example: BE MORE SUCCESSFUL. I have a daily routine that increases my success. Each day I focus my mind and visualize what I want. I increase my confidence and motivation and release past fears. I take action each day to get the results I want.
